You can also get the form by visiting the nearest first bank branch and walk straight to the customer service to request for it.

Once you complete the form and accept all the terms and conditions, then your request will be processed and you will stop receiving SMS alerts from first bank.

You can use this form to stop SMS alerts, Email alerts and monthly account statement.
